What we collect

Personal information submitted by you is collected from this website. That information is used to help us contact you, e.g. to book an appointment, by email or phone. The information is held securely by the practice and is not disclosed to anyone else (unless you give us your consent, e.g. your personal information is released as part of a patient referral).

Confidentiality Statement

The practice complies with Data Protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:

  • To provide further dental treatment for you, e.g. from hospital services or referral practices.
  • To help you get other services e.g. from a GP practice. This requires your consent.
  • When we have a duty to others e.g. in child protection cases, anonymised patient information will also be used at local and national level to help appropriate authorities.
  • Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your dental records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the dental staff.

Your Records

Under the Data Protection Act 1998 you are entitled to access your dental records should you need to. If you require a copy we can do this for you for a small handling fee to cover the costs of printing and photocopying. All requests should be made in writing to Mr Gupta and records will be made available in line with practice policy. The data controller is Mr Gupta and our registration number with the Information Commissioner’s Office is ZA088617.
